Contact DON for unexpected deaths, elopement, VA incidents, fall or incidents with sever injury  or if resident is transported out of the building 

Contact LALD for unexpected death, VA reportable events, Law enforcement at building not EMS, Facility emergencies, loss of power, fire, flood, etc

Contact Physician for :falls, med errors,change of condition, transport out of building Death (on hospice or not)
